I permit my child to participate in this Preschool Skating program. I hereby agree to indemnify and hold harmless the members, agents, instructors, supervisors, and successors of the Preschool Skating program from any and all claims, demands or causes of actions of any kind whatsoever in respect of bodily injury, death, property damage my child/children or I may suffer as a consequence of participating in this program.

Payments are due at the time of registration. $150.00/child cash, check or E-Transfer to preschoolskate@hotmail.com. Receipts will be given to you on the first day of the session. There will be a $25.00 charge for any NSF checks. Refunds will only be issued for medical reasons.
